Youth Mentoring Program

Thank you for your interest in the Youth Mentoring program. Depending on whether you are looking for a mentor or looking to be a mentor there are some forms that need to be filled out first. See below and click on the appropriate link for your situation.

It all starts by filling out the Youth Registration Form, which should be filled out by a parent or guardian.

Once the registration form above is completed have your youth complete the Youth Interest Survey so we can find the right mentor for them.

Thank you for your interest in becoming a mentor, we first ask that you review some information on what it means to be a mentor.

After reviewing the information and are still interested in becoming a mentor please fill out the Mentor Registration Form.
